Mother Nature AI: Med Tracker Is on the App Store

Med Tracker is our free iPhone app for logging doses in a tap, tracking symptoms and mood, keeping a journal, pulling in sleep and activity from Apple Health, and asking an AI how you are really doing. Here is what it does, who it is for, and how it fits with the Mother Nature AI experts.

Mother Nature AI: Med Tracker is our iPhone app for people who take medications and want a clear record of what they took and how they felt. It is free on the App Store: Mother Nature AI: Med Tracker.

This post is the long version of why it exists, what it does, and how it fits with the rest of Mother Nature AI.

Why we built it

The Mother Nature AI thesis is that everyone should have access to reliable health information and tools that actually help them take better care of themselves. We have been building the AI chat side of that thesis for a while, and we kept hearing one specific piece of feedback:

"I'm trying to keep track of my medications and I cannot find a single app that doesn't either spy on me, charge me every month, or fail to do the one thing I actually need: let me hand the data to my doctor."

When one of our engineers needed to taper off a medication under their physician's guidance, that abstract feedback became a concrete problem. They needed to track every dose, log how they were feeling, watch for side effects, and bring data to follow-up appointments. The job sounds simple. It was not.

The Apple Health gap

Apple Health has a Medications feature, and it handles simple dose logging well. What it does not do is let you export your medication history as something a doctor can read in thirty seconds. The workarounds are screenshots, the giant Health-archive XML export, or typing it all out by hand.

Third-party apps were either privacy-poor, feature-thin, or paywalled a pill reminder behind a monthly subscription. Our engineer ended up with months of doses in a notes app. That is the entire reason Med Tracker exists.

What Med Tracker does

  • Log a dose in one tap. Every medication and supplement on its own schedule. Tap when you take it and the time is recorded for you.
  • Reminders and streaks. Custom schedules for multiple medications, reminders when a dose is due, and streaks that make adherence visible.
  • Taper charts. A clear chart of your dose history, built for people stepping down a medication with their doctor.
  • Symptoms, mood and journal. A daily check-in for how you feel, with severity sliders for symptoms, a 1 to 10 mood scale with feeling tags, and a private journal, all logged next to what you took.
  • Apple Health, optional. Sleep, activity and fitness flow in when you allow it, so the whole picture is in one place.
  • AI insights. The app correlates your doses with your symptoms, mood, sleep and activity and points at patterns you would not spot in a notes app. You can also chat with an AI about how you have been doing, with all of that in front of it.
  • Support when you need it. Grounding exercises, tips, and guided breathing with calming scenes, one tap from the home screen.
  • Doctor-ready PDF reports. One tap produces a clean report of doses, symptoms and trends to bring to your next appointment.

Who it is for

Managing daily prescriptions. Several medications, different times, and a life that does not stop. Log them fast and see adherence at a glance.

Tracking supplements. Vitamins, herbs and nutrition protocols, with notes on what changed when you started or stopped. This is where Med Tracker and the Mother Nature AI experts meet: AskMN can tell you what the research says about a supplement, and Med Tracker can tell you what happened when you took it.

Tapering with your doctor. A dose-by-dose record and chart you can share, so the conversation with your clinician starts from data instead of memory.

Privacy

Everything you log is encrypted. Apple Health access is optional and read-only. Data is shared with AI services only with your explicit consent, and it is never sold. We do not use your data to train models unless you opt in, and you can withdraw at any time. The full position is in the privacy policy.

What it is not

Med Tracker is not a medical device. It does not diagnose, treat or prescribe. It tracks what you tell it and surfaces patterns. Every meaningful decision about your medication should be made with your physician.

Med Tracker does not replace Apple Health or your medical record. Apple Health is still the place for fitness, vitals and lab results. Your provider's chart is still the medical record. Med Tracker is the medication-and-symptom layer that sits next to both.

Med Tracker is iPhone only for now. It requires iOS 17 or later. An Android version is being evaluated and we have no timeline to announce.

How it fits with Mother Nature AI

The AI you talk to in the Mother Nature AI chat is good at general health questions but does not know what you take. Med Tracker knows what you take but, on its own, does not know your broader health context. Connecting the two, with your consent, is where this gets interesting, and it is the direction the whole product family is heading.

Frequently Asked Questions

What is Mother Nature AI: Med Tracker?
Med Tracker is a medication, symptom and mood tracking app for iPhone from Mother Nature AI. You log each dose with one tap, check in on symptoms and mood, keep a private journal, and optionally sync sleep and activity from Apple Health. AI insights correlate what you take with how you feel, and you can export a PDF report for your doctor.
Is Med Tracker free?
Yes. Med Tracker is free to download and use on the App Store. There is an optional Pro subscription, available monthly or yearly, for people who want more.
Does Med Tracker work with Apple Health?
Yes, optionally. If you allow it, Med Tracker reads sleep, activity and fitness data from Apple Health so the app can line up what you took against how you slept and moved. It is read-only and you choose what to share.
Can I use it for tapering off a medication?
That is one of the main reasons it exists. Med Tracker keeps a dose-by-dose record and shows a taper chart of your dose history, so the conversation with your clinician starts from data. It is a tracking tool, not medical advice, and tapering should always be done with your doctor.
Is my data used to train AI models?
Not unless you explicitly opt in. Your data is encrypted, shared with AI services only with your consent, and never sold. You can read the full policy at askmn.ai/privacy-policy.
What happened to Taper AI?
Taper AI was the original name of this app. It has been renamed Mother Nature AI: Med Tracker and expanded with mood check-ins, a journal, grounding exercises, guided breathing and Apple Health sync. If you had Taper AI installed, you have Med Tracker now.
